
A concerned mother said she warned HMP that her son was suicidal multiple times before he died in January 2024. Daniel Edwin Fielding, a father of one known affectionately as Danny, was found dead in his prison cell on January 19, 2024, just a day after he told his mum he "wanted to end it all".
At the today, June 16, his mother Margaret Farley said she had called HMP Liverpool seven times to express concerns about her son's mental health and risk of self-harm. Ms Farley said: "The calls I had from (Danny) were up and down. At times he was really upset, crying, telling me he couldn't cope, and at other times he was quite cheerful, telling me he was OK and would be out soon."
She added: "On January 18, Danny called me as usual that morning. He told me he was really down and wanted to end it all. I told him to speak to someone."
The 38-year-old was found unresponsive in his cell the next morning, with a note discovered nearby. Pathologist Dr Jonathan Medcalf found his cause of death was neck compression due to hanging.
Toxicology tests also found a breakdown of a synthetic cannabinoid, in his system.
His mother added: "Due to the trouble Danny got up to he was in prison most of his life. He was a known risk of self harm and they were aware of his mental health issues. He had made numerous attempts on his life while in prison."
She said she had called the prison on October 13, 17 and 22, December 7, 11 and 20, and January 9. Following her call on December 7 to ask staff to check on Daniel, after he told her he was "down" and had self-harmed, she said her son told her that no one had been in touch with him.
His mother said she was told to contact Safer Custody when she complained to the prison, but the call went to voicemail. She said she left a message, but did not hear back.
His mother said she was referred again on December 30 after Danny told her he had self-harmed again, and Safer Custody did not get back to her.
At the inquest, coroner Johanna Thompson told the jury how Daniel had been placed under ACCT, a process to support individuals at risk of suicide or self-harm, twice in December.

She said: "Danny had a history of problems with his mental health and some illicit drug use, and he had made attempts to end his life and to harm himself in the past. He was sadly found in his cell on the morning of January 19 2024."
Describing her son, Ms Farley said: "He was my world, my oldest of three, and was a bundle of joy. He was a sweet little boy with ginger locks and chubby cheeks. He was a cheeky little boy, always smiling and getting up to mischief as little boys do.
"Growing up, Danny ended up making friends with the wrong people and getting mixed up with the wrong crowd. He would always tell me he was sorry."
She added: "My son had a big heart and I miss him so much. Life hasn't been the same without him. He continued to suffer with his mental health, but would always put on a brave face. We have all been left numb and will try to come to terms with his loss."
