How to apply for Ayushman card
Visit a service centre: Visit your nearest Common Service Centre, Public Service Centre or UTI-ITSL centre.
Documents required: Bring identity proof (Aadhaar card, PAN card, driving license, voter ID or any other government issued ID) and your family Samagra ID.
Eligibility check: The staff at the centre will check your eligibility based on the documents you provide. If you are eligible, you can apply for your Ayushman card.
